In-Network Insurance
We are in-network with the following insurance providers:

- Medicaid (Badgercare)
Out of Network
If we are not in-network with your insurance company, we can provide you with a statement to submit to your insurance company for reimbursement.
Self-Pay Rates
For clients who pay out of pocket, a discounted rate may be offered. Fees start at $120. All fees are due at the time of service in the form of cash or credit, checks are not accepted.
Cancellation Policy
Please provide at least 24 hours notice for all cancellations. A late cancellation fee may be applied for appointments cancelled with less than 24 hours notice.
